Once your location agrees to your services - this form will provide the blueprint of all the pertinent details needed to move forward with vending placement. 
 
These are questions that can be asked at a vending proposal meeting once the location has given the ok to place your vending machine(s) within their establishment - this will assist with delivery and also future servicing of the location.
